We received a 31 years old male who presented to the emergency after been stabbed on both thorax. The left thoracic wound was site of evisceration colon. eviscerated colon perforated. patient instable. After stabilisation, he operated under general anesthesia. A bilateral drainage done,one and right stab then closed. median laparotomy discovered colic herniation through diaphragmatic coupole, continuing thorax by gastric tear. wached reduced first abdomen gentle traction. parietal cavity washed hole transverse colonic tear excised He stayed 5 days in intensive care unit. had superficial infection treated saline wet dressing for 20 days. one month hospitalisation, discharged. resumed his work 3 months is doing well at 12 postoperative. This case emphasizes rare surprise awaiting trauma surgeon.
